Medical Notes  $10.00

In general visits to Health Services are covered by provincial health cards (OHIP) or UHIP. Certain forms requested by third parties ex. drivers license physicals are not covered by OHIP and are subject to an additional charge which is determined by the complexity of the form.

Missed appointment fees are charged for no shows or cancellations with less than 24 hours notice. Fees are $10 for short appointments and $20 for long appointments
